Basic Information

Employee Identification Number
Know and use your employee ID number when contacting the Human Resources or Payroll Departments. Your employee ID number is located on the left top corner of your electronic pay statement.

Legal Name
If you are known as a first name other than your legal first name, please inform Human Resources. However, your legal name must appear on your pay records.

Address/Home Phone Number Change
Complete an Address Change Form (available from either Board Office or all schools), and submit to the Human Resources Department for processing.

Name Change
Complete a Name Change Form (available from any school or either Board Office), and submit to the Human Resources Department for processing. When submitting this form, please attach:
a) A copy of your marriage certificate, or
b) If returning to your maiden name, a copy of your birth certificate

Bank Information Change
If you need to make a change to your bank information, complete a Bank Information Change Form (available from any school or either Board Office), and submit to the Payroll Department along with a void cheque or direct deposit information form from your financial institute. Changes must be made in writing two weeks prior to the pay date.

Criminal Background Checks/Offence Declarations
Regulation 521/01 Safe Schools Act requires that all employees upon hire provide a satisfactory Criminal Background Check that includes a vulnerable sector screening. The background check must be current (within 6 months of hire date).

The Act also requires an Offence Declaration be made each year. An Offence Declaration is a means of informing the Board of any convictions that may have occurred since the original background check was submitted. You will be notified each May via the Board’s e-mail system regarding completion of the declaration. Offence Declarations are done electronically through Our Training Room.

Rating Statements:
ETFO:
A QECO rating statement must be submitted to determine your salary category placement.
Applications for a rating statement can be made by contacting the Qualifications Counsel of Ontario at:

To obtain a category change, application must be made to QECO and a revised statement submitted to Human Resources – refer to Article 9.03 regarding timelines for submitting category changes. After
receiving your request, QECO will send yo a green postcard documenting the date they received your request. This card should be forwarded to Human Resources, which will be filed in your personnel file to confirm the actual request date.
Category changes are effective the month following completion of courses. The onus is on the individual to provide Human Resources with confirmation of the completion date of courses.

OSSTF:
An OSSTF rating statement must be submitted to determine your salary category placement.
Applications for a rating statement can be made by contacting the OSSTF/FEESO Certification Department of Ontario at:

To obtain a category change, application must be made to OSSTF and a revised statement submitted to Human Resources – refer to Article 8.02.2 regarding the timelines for submitting category changes. (Notify the HR Department in writing or by e-mail, of your pending category change).
Category changes are effective the month following completion of courses. The onus is on the individual to provide Human Resources with confirmation of the completion date of courses.
